Barium Toxicity and Electrolyte Imbalance

Mechanism of Action

• Barium acts as a muscle stimulant and a potassium-sparing antagonist, but primarily it causes a massive, rapid intracellular shift of potassium.
• This profound hypokalemia (low blood potassium) is the hallmark of acute barium poisoning, which can severely disrupt cardiac function.

Cardiovascular Effects

• Acute barium toxicity frequently triggers severe hypertension initially, due to the direct stimulating effect of barium ions on vascular smooth muscle.
• As hypokalemia deepens and systemic toxicity progresses, this can rapidly transition into cardiac arrhythmias, muscle weakness, paralysis, and potentially fatal cardiovascular collapse.
